Parental/Guardian Consent and Agreement
MedicAlert's children's privacy policy is set forth below and is available at www.MedicAlert.org. As stated in the children's privacy policy, MedicAlert will not collect or use personal information about your child unless you grant your consent by agreeing to the information in this page below.

In accordance with the Children's Online Privacy Protection Act, we have adopted special procedures applicable to personal information from children under the age of 13. The following procedures applicable to children supersede any conflicting portions of our general privacy policy.

What personal information does the website seeks to collect from your child?
When a parent attempts to sign up their child, the parent will be asked to provide his or her name, date of birth, e-mail address, mailing address, day phone number and credit card information, personal healthcare information, as well as to create a user name and password. The parent will also be given the option of providing the child's evening phone number, gender, emblem product sizing information, and will be asked to either opt-in or opt-out of our mailing list.

When a child under the age of 13 attempts to submit his or her registration information, our system will reject the submission and give the child the option of having their parent enroll them in the MedicAlert service.

We will not collect or retain any personal information from a child under the age of 13 unless you agree that you are the legal parent/guardian of the child you wish to enroll. In addition we will not collect any additional information other than that which will facilitate medical care in an emergency.

How the information is Used

With whom the information may be shared
Information about children under the age of 13 whose parents have enrolled online will be used by us to notify doctors, hospitals, emergency rescue teams, and family members that the child was found wearing a MedicAlert emblem. Their condition, medications, and dosages, and any other health issues will be conveyed to the above parties. Personal and health information will also be conveyed to the responder for the purposes of treating the child in emergencies situations.

Security procedures that are in place to protect the loss, misuse, or alteration of information.
We are required by law to take reasonable security measures to protect personal information about children. Registration information submitted by a child's parent cannot be modified except by the parent/guardian themselves that enrolled the child. All such information is contained behind secured networks and is only accessible by a limited number of employees and contractors who have special access rights to such systems. All contractors have signed confidentiality agreements with us. All credit card information supplied is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our databases to be used and accessed only as described in this policy.

How you can review and delete any information collected from your child?
The legal Parent/guardian who enrolled their child with MedicAlert online may also review or modify the information about their children within their personal health record. Before disclosing information about a child, we may request that the parent provide reasonable information about the parent for identification purposes, such as the parent's name, address, phone number, credit card number, or other information.

How you can opt not to share personal information collected from your child with 3rd parties, if you choose.
Children under the age of 13 will never be asked to consent to disclosure to third parties, and their personal information will not be disclosed to any third parties other than the agencies listed above. A parent's consent to our collection and use of personal information about a child will never be treated as a consent to disclosure of that information to third parties.

If a parent wishes to terminate the child's membership at anytime this can be done by calling 1-800-432-5378 and requesting the appropriate form for termination.
